Under Medicare reimbursement regulations, when certain patients are discharged to post-acute care, the discharging hospital is paid at a reduced rate. In some cases, this reduced payment is not justified by the actual post-discharge care. These underpayments are not reflective of coding or billing errors on the part of the provider, but more commonly result from a change in the planned course of treatment subsequent to discharge.
Teaching hospitals receive a payment from Medicare for Indirect Medical Education (IME) expenses. This payment helps to defray the added costs of maintaining the teaching program. For traditional Medicare patients, the IME payment is automatically paid by Medicare when a claim is processed. For Medicare Advantage claims, however, a separate request to Medicare must be made to receive the IME payment. Hospitals make this IME payment request by submitting a “shadow bill” to Medicare. A variety of operational issues may result in the shadow bill not being submitted to Medicare.
